Web Order Form

Instructions for Use:

1.  Please visit our Customer Service area to read our Terms and Conditions of Sale including our Damaged Merchandise Return Policy and Special Order Sales prior to placing your order.
2.  Please complete all fields in the form below.  Use your Tab Key to move from field to field.
3.  After you have completely filled in the order form, use the submit button to send your order to us.  Please print a copy of the completed form for your records. 
4.  Do not click the submit button more than one time.  The order process may take several seconds depending upon the speed of your internet connection. 
5.  If your order has been successfully received by use, you will receive a confirmation page.   If you receive any message other than confirmation of your order, please notify us so we can correct the problem.
6.  When your order is received by us, we will contact you to discuss payment options and freight and delivery options.
